Print this order form out.
(Each photo goes on it's own order form. Example: if you have '3' photos to be made into canvas prints, you will fill out '3' separate order forms, '1' for each photo).
Sign the copyright line, stating you own the copyright to the photo. (Without your signature, we can not make a treasured Canvas Works of Art for you).
Enclose your check or money order for the full amount due along with the order form and your photo(s) and send to:

We shall begin work on your photos as soon as we receive them.
We always attempt to have your canvas and photos sent to you within 7 days from our receiving them, dependant upon delivery time.
